For GPs, allied health & care coordinators

Refer a patient in under 30 seconds.

A conservative, outcome-measured physiotherapy service for persistent pain, deconditioning, balance and falls risk, and stress-related physical tension, with clear referral discipline and scope-safe communication.

We triage quickly, screen and communicate within scope, and report back, typically within two business days, with outcome-measured updates that support your documentation.

Suitable concerns.

We welcome referrals where conservative, physiotherapy-led care is appropriate.

  • Persistent or recurring musculoskeletal pain
  • Reduced mobility, deconditioning and reduced function
  • Balance concerns and falls risk in older adults
  • Stress-related physical tension and movement guarding
  • Post-injury or post-surgical movement rehabilitation
  • Patients who would benefit from breath, pacing and confidence work

When to direct elsewhere.

Please direct to a GP, emergency department or call 000 for red-flag or urgent presentations, including suspected fracture, acute or progressive neurological signs, chest pain, signs of infection or systemic illness, unexplained weight loss, or any acute medical emergency.

GC Integrative provides physiotherapy within professional scope. We are not a substitute for medical assessment, psychology, psychiatry or emergency care. Where a presentation falls outside physiotherapy scope, we screen, communicate, and refer appropriately.

Common questions

Referrer FAQ

How quickly do you triage referrals?
We triage referrals promptly and contact the patient to arrange an initial assessment. Progress updates are typically returned to the referrer within two business days of each key stage.
What funding pathways do you accept?
We accept referrals from clients using private funding, Support at Home, CHSP, NDIS, facility arrangements, and insurer, WorkCover or CTP pathways. Please indicate the funding pathway on the referral form where known.
What outcome measures do you use?
We conduct a whole-person assessment that establishes baseline outcome measures and goals. Relevant outcome data is included in the assessment summary and progress updates returned to the referrer.
How do you communicate progress back to referrers?
Referrers receive an assessment summary with findings and goals, relevant outcome measures at baseline and re-assessment, progress notes where appropriate, a discharge or maintenance plan, and escalation recommendations where needed — typically within two business days.
Can you see NDIS or Home Care Package clients?
Yes. We work with clients on NDIS, Support at Home, CHSP and Home Care Package arrangements. Please select the relevant funding pathway on the referral form so we can triage appropriately.
