If you are a lawyer or insurer case manager

Accurate documentation, a clear treatment rationale and appropriate communication with the client, the lawyer and the case manager.

  1. Download the Insurance Claim Referral Form (one A4 page, fillable).
  2. Complete the client details: name, date of birth, phone, address, accident date, claim type, insurer and claim number.
  3. Attach the relevant medical certificate (see note below).
  4. Email the form and attachments to info@gcintegrative.com.au with appropriate client consent.
  5. We reply to confirm availability for the address and the next step.

Where we consider home visits. North Gold Coast, Brisbane and Brisbane North enquiries are considered case by case. Capacity is checked for each address before a referral is accepted, and some addresses may not be serviceable. Toowoomba is not currently served. After-hours visits by arrangement.

The same physiotherapy discipline as clinic care, delivered where the client is, and documented so everyone involved can follow the reasoning.

Assessment

History, functional baseline, safety screen and the goals that matter to the client’s daily life and work.

Plan with rationale

A written plan that explains why each part of treatment is proposed and how progress will be measured.

Home rehabilitation

Graded movement, strength and function work in the home, adjusted as capacity returns.

Reports and review

Progress reports at agreed intervals, then a discharge or maintenance plan when goals are met.

Clinical reporting to referrers is typically returned within two business days of each key stage, in line with our referral pathway.

Who pays.

CTP (motor-vehicle) claims are charged at our standard rates and invoiced against the claim. For an accepted WorkCover Queensland claim we invoice WorkCover directly under their table of costs. Approved treatment carries no out-of-pocket cost to you.

WorkCover appointments are scheduled at 45 minutes for the initial assessment and 30 minutes for subsequent consultations. Where a longer program is clinically indicated, we seek insurer approval first.

If a claim is pending or declined, we will tell you before treatment starts and agree how the service is funded.

Scope and boundaries. GC Integrative provides physiotherapy within AHPRA scope of practice. Funding arrangements are confirmed before treatment. Do you treat people with CTP or WorkCover claims at home?

How quickly do you reply to a personal injury enquiry?
We will contact you to discuss your enquiry and arrange a suitable time. Submitting the form does not book an appointment. Treatment timing depends on capacity and on the funding arrangements being confirmed.
What documentation do lawyers and case managers receive?
With the client's consent, GC Integrative provides an initial assessment summary with a functional baseline, a treatment plan that explains the rationale for care, progress reports at agreed intervals, and appropriate written or verbal communication with the client, their lawyer and the insurer case manager. Clinical reporting to referrers is typically returned within two business days of each key stage.
How does a lawyer or case manager refer a client?
Download the Insurance Claim Referral Form, complete the fillable fields, attach the relevant medical certificate (for CTP referrals, the CTP Medical Certificate), and email the form to info@gcintegrative.com.au with appropriate client consent. Referrers can also use the online referral form on the For Referrers page.
Do I need an accepted claim or a lawyer before filling in the form?
No. You can request an introductory call if you are unsure about the next step. Select “Not sure” where needed. The form does not decide your eligibility or submit a claim.
Is the phone consultation free?
The introductory 5–10 minute phone consultation is free after you complete the form. There is no obligation to purchase treatment or use a suggested lawyer. Any further services and fees are discussed separately.
What happens after I complete the form?
We receive your enquiry and contact you to arrange the call. We discuss your needs and possible next steps. A lawyer introduction is optional and requires separate permission before information is shared.
Can I complete the form in another language?
Use the language selector for the languages currently available. You can separately tell us your preferred spoken language and whether you need an interpreter. We confirm call arrangements with you.
