GC Integrative is committed to protecting your privacy. We handle personal and health information in accordance with the Privacy Act 1988 (Cth) and the Australian Privacy Principles (APPs). The entity responsible for managing personal information collected through this website and our practice is GC Integrative Physiotherapy & Wellness.

What we collect

We collect personal information that you choose to provide, which may include:

How we use it

We use your information to respond to your enquiry, to provide and coordinate your care, and to communicate with you about your appointments and treatment. We only use your information for the purposes for which it was collected, or for closely related purposes you would reasonably expect.

For CTP and WorkCover patient enquiries, we use the answers you provide to prepare for a free 5–10 minute introductory phone consultation and discuss possible next steps. Completing this form does not subscribe you to marketing. We ask for your separate permission before sharing information with a named law firm for an optional introduction.

Health information

Health information is collected and stored with care and treated as sensitive information. We share health information with your GP, your referrer or your wider care team only where it is appropriate to do so and, where required, with your consent.

Storage & security

We take reasonable steps to protect the personal and health information we hold from misuse, interference, loss, and unauthorised access, modification or disclosure. This includes secure handling and storage of records, and limiting access to authorised people only.

Disclosure

We do not sell personal information. We disclose personal information only as needed to provide your care, to people you have authorised, or as required or permitted by law.

Cookies & analytics

This website may use Google Analytics 4 (a service of Google LLC) to collect aggregated, statistical information about how visitors use the site — such as pages viewed, approximate region, device type and how you arrived here — so we can understand what is useful and improve the experience. Where analytics is active, it uses cookies or similar identifiers; IP addresses are truncated/anonymised where the platform supports it, and the information collected may be processed by Google on servers outside Australia. We do not use analytics data to identify you personally.

You can control or disable cookies through your browser settings, or opt out of Google Analytics specifically via Google’s browser opt-out tools; some features of the site may not work as intended if cookies are disabled. Kajabi (newsletter)

The newsletter sign-up in our website footer is an embedded form provided by Kajabi (Kajabi, LLC, United States). Because the form is loaded from Kajabi’s servers, Kajabi and its infrastructure providers set cookies in your browser when the form loads. In our testing these are: _kjb_session (keeps the form session together), _csrf_token (a security token that protects the form against cross-site request forgery), __cf_bm and _cfuvid (Cloudflare bot-management and rate-limiting cookies that tell automated traffic from human traffic), and AWSALBTGCORS (an Amazon Web Services load-balancer cookie that keeps your requests going to the same server). They are set by Kajabi and Cloudflare, not by us, and we do not read them.

To keep this to a minimum, the newsletter form is not loaded when a page opens. It loads only once you show intent — by scrolling past about halfway down a page, by clicking, tapping or typing, or by scrolling the newsletter box itself into view. If you leave a page before then, no Kajabi cookie is set. Pages that collect sensitive answers, such as the CTP and WorkCover patient enquiry form, never load it at all.

If you subscribe, the name and email address you enter are submitted to and stored by Kajabi in the United States, and are used to send you GC Integrative updates. You can unsubscribe from any newsletter email. Kajabi’s handling of that information is governed by its own privacy policy at kajabi.com/policies/privacy. You can also block or delete these cookies through your browser settings; the newsletter form may then not work.

Access & correction

You may request access to the personal information we hold about you, and you may ask us to correct it if it is inaccurate, out of date or incomplete. We will respond to your request within a reasonable period.

We retain personal and health information for as long as required to provide care and to meet our legal obligations. Health records are generally retained for a minimum of seven (7) years from the date of last service or, for a person who was a minor at the time, until they reach 25 years of age, consistent with Australian health-records requirements.

Website forms and overseas processing

Our website forms use Formspree to receive enquiries. This includes the CTP and WorkCover patient enquiry form. Formspree hosts its services with Amazon Web Services in the United States, so the information you submit through these forms is processed and stored overseas. Submitted information is available through the clinic's Formspree account and may also be included in email notifications, depending on the form's notification settings.

Before submitting the patient enquiry form, please read its collection notice and choose whether to consent. Provide only information relevant to your enquiry; do not include identity documents, payment card details or medical reports. You can contact us to ask about our information-handling arrangements or another way to make an enquiry. Withdrawing consent after submission does not automatically delete a received request; contact us and quote the form reference so we can respond to your request, subject to applicable record-retention requirements.

If you are not satisfied with how we have handled your personal information, you may lodge a complaint with the Office of the Australian Information Commissioner (OAIC) at oaic.gov.au.
