Provider / care coordinator pathway
Simple to engage. Clean on compliance.
We work as a sub-contracted allied-health (physiotherapy) provider. You hold the funding and care plan; we deliver the physiotherapy and invoice you. The participant never pays GC Integrative directly for a funded service.
You refer / contract
Send a referral or set up a service agreement. We confirm funding and suitability.
We assess & deliver
Whole-person assessment using the Physio-Qi Method and GC Health Model two-domain framework, goal-based care plan and physiotherapy-led care, in-home or on-site.
We report & invoice you
Outcome-measured updates and clean documentation. We invoice the provider; the participant isn’t privately charged.
